Type "dentist near me" into Google and you get a page of options: a map with a few pins, some ads, and a list of links. The customer scrolls, compares, and clicks. That page is what local SEO fights over — your position on the list.

Ask ChatGPT "Who's a good dentist in Dallas?" and something different happens. There is no long list of links. The assistant writes a short answer that names a few specific businesses and explains why. In our Dallas measurement, the average AI answer named about six practices. If you are one of them, you just got a personal recommendation. If not, the customer may never learn you exist.

That is the core difference. Local SEO is about ranking on Google. AI visibility is about whether assistants like ChatGPT, Gemini, and Perplexity actually say your name when someone asks who to go to.

What Each System Rewards

Google's local results lean on signals most owners already know: your Google Business Profile, how close you are to the searcher, your reviews, and how well your website matches the search.

AI assistants build their answers another way. They retrieve text and rewrite it, so what they reward is text that states a checkable fact — on your own pages first, then repeated by the directories, review platforms, "best of" articles and local press they also read. Yext's October 2025 analysis of 6.8 million citations found Gemini takes 52.15% of its citations from brands' own sites and ChatGPT 48.73% from third-party listings (Yext); Yext sells listings management, so treat its listings-heavy framing accordingly and note that even it measured the brand's own pages at roughly half the answer.

Nobody outside these companies knows the exact recipe, and it keeps changing. But the pattern is steady enough to act on: local SEO is about a position on a page of links, while an AI answer is assembled out of sentences somebody wrote. If nobody — including you — has written the fact a customer is asking about, no amount of ranking puts it in the answer.

Why Ranking #3 on Google Doesn't Mean ChatGPT Names You

Because the inputs differ, the results can split hard. A practice can have a polished website and a solid spot on the map and still never appear in an AI answer — because the articles and directories the AI reads don't feature it.

Real numbers make this concrete. In July 2026, we measured 262 dental practices across the Dallas metro area, using 30 customer-style prompts — best-of, comparison, attribute, and urgent-need questions — each asked across ChatGPT, Gemini, and Perplexity. From 138 valid answers, we parsed 879 recommendations. The result: 177 of the 262 tracked practices — 68% — never appeared in a single answer. The average visibility score was 1.6 out of 100. The best score in the metro was just 27.0.

These are real, operating practices. The benchmark didn't test their Google rankings — and that is exactly the point. AI visibility has to be measured on its own, because it does not follow from a good-looking search presence.

A published example shows the same gap for a single business. A real Snapshot for Dentistry of Miami came back with an AI Visibility Score of 3.9 out of 100, while the top competitor was recommended 6.5 times more often — 39 mentions to 6.

Where Local SEO and AI Visibility Overlap

The good news: these two are not rivals. They share a foundation, and effort in one often helps the other.

The overlap is why a business with strong SEO fundamentals usually isn't starting from zero on AI visibility. But the last mile is different: SEO tunes your site to rank. AI visibility work makes the facts about you retrievable — stated on your pages in plain text, then said the same way everywhere else the engine looks.

If your business is new or your Google presence is thin, start with local SEO basics: a complete Google Business Profile, steady reviews, a clear website. Google still drives a large share of local discovery, and those same basics feed your AI visibility later.

If your Google side is already healthy, AI visibility is the likelier blind spot — and the one growing quietly. More and more customers ask an assistant directly and act on the short answer they get. Competitive local fields feel this first: dentistry, med spas, plastic surgery, and aesthetic clinics, where an AI answer names a handful of businesses and everyone else is simply absent.

Either way, measure before you spend. An AI visibility audit shows whether assistants mention you at all, who they recommend instead, and which sources they rely on. From there, improving your odds of getting recommended by ChatGPT becomes a matter of closing specific, named gaps — not guesswork.

The Honest Caveats

No one can guarantee that an AI assistant will recommend your business. Anyone who promises that isn't being honest. AI answers shift with the exact question asked, the competition in your market, and the behavior of each model.
